Patient Information and Resources

At Parkside Physical Therapy, we strive to provide superior treatment, care, and education regarding your musculoskeletal issues, and we promise do so with excellent customer service at all levels of your experience with us. In order to get off to an efficient start, you may download the following patient information documents (#1-3 below), fill them out ahead of time, and bring them with you to your first visit. This will allow you and your therapist additional treatment time together. These forms are also available at our clinic, and you may wait until your first visit to fill them out. In this case, please arrive 15 minutes prior to your scheduled appointment time in order to complete this paperwork.

  • The physical therapy prescription form from your referring practitioner.
  • Your current medical insurance card, or, in the case of injuries sustained in a vehicle accident, please bring your auto insurance information, including policy number and the telephone number of your insurance company.
  • The 3 patient information documents (see above). If you choose to wait until arriving at our office to fill out this paperwork, please arrive 15 minutes prior to your scheduled appointment in order to allow extra time to complete these.
  • Comfortable clothing. You will probably feel most comfortable in non-restrictive clothing that will allow you to bend and stretch. We do have shorts and tee-shirts available for your use, but if you have some favorite exercise clothing, bring it along or wear it to your treatment sessions.
  • Please check your medical insurance policy, or call us at (509) 323-0066 (preferably prior to your first visit), and we can answer this question for you. Washington State law allows physical therapy treatment on a “direct access” basis, which means that under Washington State law, you can access physical therapy treatment from a licensed physical therapist without a doctor’s referral. However, insurance companies differ on whether or not they will reimburse for physical therapy treatment without a written referral. Some insurance policies, such as those offered through Medicare and Medicaid, require a written referral for physical therapy from a doctor, nurse practitioner, or physician’s assistant. Other policies may not have this requirement. If you’re unsure what your policy requires, please call our office at (509) 323-0066, and we will be happy to help you find out.
  • At Parkside, we contract with virtually all health insurance contractors. (In the case of Group Health, we are only able to accept Group Health Options at this time.) We will be happy to contact your insurance company for you to get the specifics regarding your policy – just give us a call!
  • You certainly can at Parkside! This is an excellent and appropriate way of utilizing an HSA, and it can save you on out-of-pocket expenses related to physical therapy treatment. If you have an HSA that utilizes a “debit card” type of payment system, please bring it with you to your treatment sessions.
  • If you have ANY questions regarding your insurance coverage, copayments, or billing as related to physical therapy, please contact us at (509) 323-0066, and we will help you find an answer.
  • Your first treatment session (we call it your “Initial Evaluation”) will last approximately 1 hour. Thereafter, your sessions will most likely be 45-60 minutes in length. Your therapist will discuss this with you in more detail during your first and subsequent sessions.
  • At Parkside, we make every attempt to schedule you with the same physical therapist for each treatment session, as we believe the continuity of care is important for both you and your therapist. We do not employ physical therapy aides or assistants, and your physical therapist will be working directly with you for the entirety of each session, including during any exercise portion of your treatment.
